CUMYL-4CN-BINACA (also known as CUMYL-CYBINACA or SGT-78) is a synthetic cannabinoid based on indazol-3-formamide that has been marketed online as a specialty drug. It is a potent agonist for the cannabinoid receptors CB 1 and CB 2, with in vitro EC 50 values of 0.58 nM and 6.12 nM, respectively. In mice, CUMYL-4CN-BINACA produces hypothermia and induces convulsion through the action of the CB 1 receptor, and anecdotal reports suggest an active dose of about 0.1 mg in humans. CUMYL-4CN-BINACA is metabolized to produce cyanide, raising concerns about liver toxicity. There has been one reported case of hyperthermia, rhabdomyolysis and kidney failure associated with its use.
